Individualised Exercise Training Enhances Antioxidant Buffering Capacity in Idiopathic Pulmonary Fibrosis

Exercise training is recommended for patients with idiopathic pulmonary fibrosis (IPF); however, the mechanism(s) underlying its physiological benefits remain unclear. We investigated effects of an individualised aerobic interval programme on exercise capacity and redox status in IPF patients. were recruited prospectively to 8-week, twice-weekly cardiopulmonary test (CPET)-derived structured responsive (SRETP). Systemic was assessed pre- post-CPET at baseline following SRETP completion. An age- sex-matched non-IPF control cohort comparison only. At baseline, (n = 15) had evidence increased oxidative stress compared controls as judged by; plasma reduced/oxidised glutathione ratio (median, 1856 vs. 736 p 0.046). Eleven completed (median adherence 88%). Following completion, there a significant improvement via constant work-rate endurance time (+82%, 0.003). This accompanied by post-exercise (in favour antioxidants) serum total free thiols increase, +0.26 μmol/g protein 0.005) concentration (+0.73 μM 0.03), well decrease lipid peroxidation products (−1.20 0.02). circulating nitrite concentrations significantly lower (−0.39 0.04), suggestive exercise-induced utilisation.
